Please read and agree to the following:
2.) I further understand and agree that in admitting my dog named above to the Center, Positive Approach has relied on my representation that my dog(s) is/ are in good health, have not been ill with any communicable disease with the last 30 days, and have not harmed or shown aggressive or threatening behavior towards any person or any other dog.
3.) I further understand and agree that Positive Approach Training Programs, its staff and its volunteers will not be liable for any problems that develop, provided reasonable care and precautions are followed, and I hereby release them of any liability of any kind whatsoever arising from my dog(s) attendance and participation at the Center.
4.) I further understand and agree that any problem that develops with my dog(s) will be treated as deemed best by the staff of Positive Approach Training Programs, in their sole discretion, and that I assume full financial responsibility for any and all expensed involved.
5.) I understand that there are risks inherent in any large group of dogs interacting with each other and that even under close supervision injuries can occur. If it is known which dog caused an injury, that dog's owner will be liable for all expenses incurred. If it is not known which dog caused an injury, each owner is responsible for their own expenses!
6.) I have read and understand the Daycare policies as stated on the Daycare Policies page .
7.) I further understand that my dog could potentially be used as a greeting dog in the temperament and evaluation process of all potential daycare dogs.
